Waivers

Our goal is to give your family a place to call home…where your athlete learns, flips, and evolves into the person we all strive to be!

Please provide a written signature to the following documents below prior to the start of class or program/event.

1. A Participation Waiver is to be signed for each athlete.
2. An Appearance Release form Please bring this form whether you accept or decline.
3. A Parent/Guardian Liability Waiver is to be signed by each adult entering as a parent participant with their child, 3 years of age or younger.
